Phlebotomy Technician Certificate Program (6-8 weeks)
APPLICATION FORM
Phlebotomy is to obtain blood speciments from clients for the purpose of laboratory analysis. The focus of this program is to
provide the students with the comprehensive skills needed to perform vein punctures competently and safely. The phlebotomy
program provides the theoretical knowledge, practical skills and attitudes required for a phlebotomy position.
After completing the program, there is a NATIONAL CERTIFICATION EXAMINATION that students have the option to complete. There is a fee
to take the exam, payable to the National certification center. Application for the exam is also at Mentee Medical Institute.

Perform a minimum of 100 successful vein punctures. Wear nursing uniforms to Clinical / Externship. Clinical hours will be done in a healthcare setting. Three weeks BEFORE clinical, each student must provide the school with the following: 1. Proof of student malpractice insurance 2. TB skin test from local health department 3. Hepatitis vacine from local health department 4. Background record check |
